Care Partner – Aged Care Support at Home

Location: Greenacre, Sydney NSW (On-Site)

Employment Type: Full-Time

Salary: $40-$45/hour + super

What You’ll Do: As a Care Partner, you will play a key role in supporting elderly participants across South-west Sydney. You will assess and coordinate individual care needs, develop person-centred support plans, manage budgets and schedules, and ensure clients receive the services they need to live independently at home.


Key Responsibilities:

  • Conduct comprehensive participant intake assessments to understand individual needs, goals, and preferences

  • Develop, implement, monitor, and review participant Aged Care Support at Home budgets and schedules of support

  • Collaborate with participants, families, and healthcare providers to develop person-centred care plans

  • Coordinate and schedule in-home support services including personal care, housekeeping, meal preparation, and social activities

  • Monitor participant wellbeing and adjust care plans as required

  • Act as the primary point of contact for participants and their families, providing ongoing support and guidance

  • Maintain accurate client records and ensure compliance with relevant regulations and policies

  • Work collaboratively with the wider team to drive continuous improvement and enhance service quality


What You Bring:

  • Experience as a Care Manager within Aged Care Support at Home or the previous Home Care Package program

  • Strong knowledge of the Aged Care Act 2024, Aged Care Rules 2025, and Aged Care Strengthened Standards 2025

  • Relevant experience in aged care or disability support, with a demonstrated commitment to person-centred care

  • Strong communication and interpersonal skills, with the ability to build trust with clients and families

  • Excellent organisational and time-management skills, with the ability to prioritise and manage multiple responsibilities

  • Empathetic and compassionate approach with a genuine commitment to improving people's lives

  • Relevant qualifications such as Certificate III in Individual Support or equivalent

  • Current Australian driver's licence and access to a reliable vehicle

  • Arabic language skills are preferred but not essential
